REQUIREMENTS AND PROCEDURE FOR THE APPLICATION OF MACHINE READABLE PASSPORT (MRP) AT THE PHILIPPINE EMBASSY IN PRAGUE

Important:

• The Embassy no longer issues the old regular green passport. It will only process the application for MRP.
• Actual issuance of the passport will be done by the Department of Foreign Affairs (DFA) in Manila.
• Processing time of the new Machine Readable Passport (MRP) is from three (3) to four (4) weeks from the day the application is filed. Due to the longer processing time, it is advised that all Philippine passport holders apply for renewal at least six (6) months prior to the expiration of their existing passport.
• Unlike the old regular passport, the MRP cannot be amended. Therefore, every application will be treated as a new application for an MRP.

PASSPORT RENEWAL
Requirements:

1. Duly accomplished passport application form (form available at the Embassy);
2. Two (2) passport-size photographs (colored with ROYAL BLUE background); and
3. Previous passport (original and photocopy of data page).

Married women who want to apply for a change of status must apply for registration of marriage first.

FOR LOST OR DAMAGE PASSPORT
Requirements:

1. Duly accomplished passport application form (form available at the Embassy);
2. Two (2) passport-size photographs (colored with ROYAL BLUE background);
3. Written Police Report [applicants should be advised to report the loss of their passports to the police and to bring the report to the Embassy when applying for replacement] translated in English;
4. Affidavit of loss;
5. Photocopy of the data page of the lost passport and visa, if available; and
6. Other identification documents to establish the identity of the applicant.

NEW BORN
Additional requirements:

1. Duly accomplished passport application form (form available at the Embassy);
2. Two (2) passport-size photographs (colored with ROYAL BLUE background);
3. Original and 4 photocopies of applicant/infant’s registered Birth Certificate (translated in English);
4. Duly accomplished Report of Birth;
5. Four (4) photocopies of both parents’ valid passports; and
6. Four (4) photocopies of Marriage Certificate (translated in English)
